Description

Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician's plan of treatment, taking into account individual patients special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential.

Required Qualifications:

• Bachelor's Degree in Physical Therapy; from an accredited institution or



• Master's Degree in Physical Therapy; from an accredited institution



• Upon hire: Oregon Physical Therapist License



• Within 30 days of hire: National Provider BLS - American Heart Association .



• 1 year Clinical experience in a healthcare setting, preferably home health, or active enrollment in the Providence Benedictine postgraduate preceptor program.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses.
